For the above named player and in consideration of participating in the X-Roads Warriors Football Program, I hereby release, acquit, forever discharge, indemnify and hold harmless organizers including all coaches, X-Roads Warriors FC, the BFA, all sponsors, operators, facilities used and their respective officers, directors, employees, agents and representatives of and from any and all claims, demands, actions, or causes of action, liabilities, or damages arising out of or in any way related to my participation in the X-Roads Warriors FC programs and any related activities. I further certify that I have no known or existing medical conditions that prevent me from participating in physical exercise and activities fully. In the event of injury or illness I hereby grant authority to be qualified physician to render such medical treatment as said physician deems reasonable and necessary, and we also accept responsibility for any expenses involved with such illness, injury and treatment. I also hereby give permission for X-Roads Warriors FC to use images/videos of my child for promotional materials on Facebook or any other social media Platforms.
